Florida SHOTS
What is Florida SHOTS? Florida SHOTS
is a statewide centralized electronic immunization registry
that is authorized by section 381.003, F.S. The purpose
of Florida SHOTS is to help parents and health care
providers keep track of immunization records.
By including your child in the registry, you and your
physician will be able to:
Have access to an automated, updated immunization
history for your child through the convenience of
Florida SHOTS
Be able to request an automated immunization form,
needed for school or day care entry and attendance,
from your private provider or managed care provider
Receive an automated reminder from Florida SHOTS
records for immunizations that are due
Have an automated notification to providers that
your child has experienced an adverse event to a
specific shot or that your child has a contraindication
to a vaccine
Florida is an Opt-Out state. This means
that unless specified by completing form
1478 (opt-out), your record or your child's record
may be added to the registry. You do not need
to complete the form to participate. The form is to
provide notification and you child will be included
if you do nothing. To choose not to participate,
simply fill out the bottom part of form
1478 (opt-out), and mail it to the specified address.
